Clinical Pharmacists

Hurley Group has employed a team of Clinical Pharmacists to work across all its surgeries, promoting the safe and effective use of medication. The Clinical Pharmacists support patients through face to face clinics or remotely via e-consultations and optimise the use of medicines by undertaking:

 

  • Structured medication reviews
  • Monitoring of high risk drugs
  • Long term condition clinics such as asthma, hypertension and diabetes etc.
  • Minor ailment clinics
  • Medicines Management work commissioned by the CCG
  • Drug recall and switches as advised by the Medicines and Healthcare products Regulatory Agency
  • Medicines Reconciliation against clinic letters and hospital discharge summaries
  • Resolving medication queries from Secondary Care and Community Pharmacies
  • NHS contracted work such as QOF and Primary Care Network Direct Enhanced Services

 

A collaborative bid by Hurley Group & Kings College Hospital was successful as part of wave 2 of the NHSE GP practice pharmacist programme, with the vision to integrate clinical pharmacists into general practice and hospital. These Clinical Pharmacists working across both hospital and general practice have been able to identify gaps in the care pathway across primary and secondary care and make provisions for their patients where a gap may impact on their care.
